Appointments
When you call our office to make an appointment we will make every effort to accommodate your schedule, but please understand we have guidelines we follow in order to maintain efficiency and keep our fees as low as possible. Certain times of the year are also busier due to seasonal variations and may require longer waits for some appointment types. If you would like to schedule an appointment information you will need the items listed below or your can click here to fill out a request form and someone from our office will contact you to within two business days.
In order to schedule your appointment promptly, please give the following information:
- Patient’s first and last name
- Home, cell or work number
- Date of birth
- Have you been seen previously by any of our doctors?
- Type of consultation
- Office location
- Name of doctor you are requesting to see, if you have a specific request
- Referring dentist or orthodontist (referral slip and x-ray)
- Medical and/or dental insurance information
